What if I am under under 18 how make art? Well, if your under 18 you
can't sign a waiver, a parent has to.
There are 3 options for you.
1. The person under 18 has a State issued ID and matching address as the
parent.
So Peggy Sue who is 16 has a Indiana ID and the parent is Megan Sue both
live at 1234 Main Street, then Megan can sign a waiver for Peggy.
So Peggy Sue who is 16 has a Indiana ID and the parent is Megan Sue both
live at 1234 Main Street, but dad brings in Peggy and the address DON'T
match then Megan AND Dad would have to sign the waiver for Peggy
OR
2. Fill out the notary, If you have a bank it can be done for free.
OR
3. We verify they are your son or daughter, as it takes away from our
time, we charge $20.00 PER minor to accept or decline paperwork provided, what we
need is: (Can not be copies)
Documents Needed:
- Parents ID
- A piece of mail with the child's name matching the address on the
parents ID
- Birth certificate or other form of documentation showing matching child's name
& parents name.
Parent - A parent is the person (s) who have legal custody of the
